As a patient or family, you are responsible for.....
- Providing complete and accurate information
- Notifying Twin Oaks Home Care about any changes in your health
- Participating in planning and revising your home care program
- Treating our personnel with consideration, courtesy, and respect.
(Understand that you will receive 2 to 3 employees for each shift you request. The 4 shifts we provide are day, evening, night, and weekend. If someone requests day and evening shifts during the week this would mean there would be between 4 to 6 employees providing their care. This is done to ensure dependability of care. If you refuse new or additional employees you may not receive care for the shifts you refuse.)
- Providing a safe environment for your care.
(Understand that Twin Oaks Home Care, Inc. does not provide "dead lift transfers". Patients who cannot assist with transfers must use a sliding board, a hoyer lift, or remain in bed during care. You also need to understand that the use of proper medical equipment will be determined by the Twin Oaks Home Care, Inc. Registered Nurse, in order to provide safety measures to it's employees.)
- Providing sufficient and appropriate medical supplies and equipment for your care. (including hand soap, paper towels, and cleaning supplies) (We will assist in obtaining these if necessary)
- Taking prescribed medications as authorized by your physician, and notify your physician of any side effects.
(A pill box must be used for all consistent medications. Patients, families, and Skilled Registered Nurses are responsible for the set up of their pill box.)
- Notifying Twin Oaks Home Care if you will not be available for a scheduled visit at least 24 hours prior to the scheduled visit. (Example: doctors appointment, emergency room visit, hospitalized, etc.)
- Providing accurate insurance and financial information, including any changes in coverage.
- Notifying Twin Oaks Home Care of the existence of, and any changes made to , Advanced Directives. (Example: Power of Attorney, Living Will, etc.)
- Notifying a Twin Oaks Home Care Supervisor immediately of any problems or dissatisfaction you may have with the services you are receiving.
(Understand if you request a certain employee not come back to your residence that schedules are completed 1 week in advance. We may need to finish the current schedule to provide you with a new employee, or we may need to simply decrease that employee's time at your home over a period of 2 to 3 weeks to phase them out of your home. However, if it is a matter of safety that employee will be removed from your home immediately.)
- Notifying a Twin Oaks Home Care Supervisor or Secretary directly of any schedule changes.
