To choose your home care provider

 To participate in all aspects of your home care program.

 To request and receive all information regarding your medical records.

 To refuse any, all, or partial treatments or services and the possible consequences of your refusal.

 To receive services without discrimination of age, race, religion, sex, disability, color, creed, sexual preference, or national origin.

 To provide confidentiality of all records, personal information, and communications about your health,  social, and financial circumstances, and will only be released when required by law or when authorized in writing by yourself, your court appointed designee or your insurance company.

 To have a 10 day prior notification if services would be transferred or terminated, unless the employee safety is at risk, then services would be terminated immediately.

 To initiate grievances and make suggestions in any service without fear of reprisal or discrimination.

 To have your person and property treated with dignity, respect, and consideration.

 To be fully informed of all charges, changes in all charges, and policies regarding payment of services and to receive information orally and in writing within 30 days in which the home care provider becomes aware of these changes.

 To be informed of the extent to which payment may be expected from any other payer known to the home care provider.

 To formulate an advanced directive, and be informed that your care will not be based on whether or not you have an advanced directive.

 To be informed of the PA Department of Health toll free hotline # 1-800-254-5164, and hours of operation (8:30am - 5:00pm; Monday-Friday).

To be informed of the Ombudsman Program with the local Area on Aging # 1-888-300-2704

 To be able to verify agency licensor requirements and compliance at the:

 To receive assistance from the Office of Long Term Living for unresolved concerns regarding their programs at # 1-800-757-5042 or 1-866-286-3636.
